Every November, hundreds of thousands of Canadians migrate south for 3–6 months. A shocking number still believe their OHIP, MSP, RAMQ, or other provincial health card travels with them. It does — but only in the most technical sense. Here is everything a Canadian snowbird must know about medical insurance in Mexico before departure.
The Provincial Plan Reality: Pennies on the Dollar
Provincial plans do reimburse emergencies outside Canada — but only up to what the equivalent service costs inside that province. Private Mexican hospitals charge in USD and operate for-profit; provincial fee schedules cover typically under 10% of a Mexican hospital invoice. Example: a $20,000 USD hospital bill might come back with a $2,000 CAD reimbursement from your province. The remainder is yours.
The Absence-Limit Clock Starts the Day You Leave
To keep provincial coverage valid, you must reside in your province a set number of months each year — Ontario allows roughly 212 days (7 months), Quebec 6 months, BC 6 months, Alberta 6 months, and so on. Exceed the limit and you can lose provincial coverage, which also voids most private travel policies. Snowbirds should track every day outside Canada and buy multi-month insurance with extension options if stays grow.
Pre-Existing Conditions: The #1 Denial Reason
Travel insurers cover pre-existing conditions only under specific rules: acute-onset benefits (Trawick and IMG offer these on short-term plans) or underwritten riders (GeoBlue expat plans). Never hide a medication — undisclosed conditions are the single most common reason claims are denied.
Direct-Pay Networks: Your Best Friend in an Emergency
Choose an insurer with contracted Mexican hospitals that bill the carrier directly. GeoBlue's BCBS network is the deepest in Mexico; IMG and Trawick also maintain contracted networks. Our destinations guide lists direct-pay hospitals for 30 Mexico cities.
The Six-Point Canadian Snowbird Checklist
- Buy insurance at least one day before departure (you cannot buy once you arrive)
- Pick a plan covering acute onset of pre-existing conditions if you have a medical history
- Verify the policy extends to your full planned stay (IMG extends to 24–36 months)
- Save the carrier's 24/7 assistance number in your phone and as a screenshot
- Count your absence days against your province's limit
- Keep your provincial coverage active for the entire policy period
